Indications/Uses
Serious infections caused by susceptible microorganisms in the following conditions: Lower resp tract infection, complicated & uncomplicated UTI, intra-abdominal infections, gynecologic infections, bacterial septicemia, bone & joint infections, skin & skin structure infections, endocarditis, polymicrobic infections.
Dosage/Direction for Use
Fully susceptible organisms including gram +ve & gram -ve aerobes & anaerobes Mild: 250 mg 6 hrly. Total daily dose: 1 g. Moderate: 500 mg 8 hrly; 500 mg 6 hrly. Total daily dose: 1.5 g; 2 g. Severe life-threatening only: 500 mg 6 hrly. Total daily dose: 2 g. Uncomplicated UTI 250 mg 6 hrly. Total daily dose: 1 g. Complicated UTI 500 mg 6 hrly. Total daily dose: 2 g. Moderately susceptible organisms, primarily some strains of P. aeruginosa Mild: 500 mg 6 hrly. Total daily dose: 2 g. Moderate: 500 mg 6 hrly; 1 g 8 hrly. Total daily dose: 2 g; 3 g. Severe life-threatening only: 1 g 8 hrly; 1 g 6 hrly. Total daily dose: 3 g; 4 g. Uncomplicated UTI 250 mg 6 hrly. Total daily dose: 1 g. Complicated UTI 500 mg 6 hrly. Total daily dose: 2 g. Max total daily dosage: not exceed 59 mg/kg/day or 4 g daily. Childn 3 mth Non-CNS infections 15-25 mg/kg/dose 6 hrly, 4 wk-3 mth 25 mg/kg 6 hrly, 1-4 wk 25 mg/kg 8 hrly, <1 wk 25 mg/kg 12 hrly. Doses ≤500 mg should be given by IV infusion over 15-30 min. Doses >500 mg should be given by IV infusion over 40-60 min.
Contraindications
Hypersensitivity. Patients w/ meningitis.
Special Precautions
Hemodialysis. Not for direct infusion. Pregnancy & lactation.
Drug Interactions
Probenecid, ganciclovir, aminoglycosides.
